    The State of California has specific health requirements that must be met prior to starting Kindergarten, 1st and 7th Grades. They are listed below: Prior to registering for Kindergarten, you will need:

    • Four (4) Polio immunizations (3 doses if at least one was given on or after 4th birthday)
    • Five (5) DTP immunizations (if last dose was given before 4th birthday, one more dose is required)
    • Two (2) MMR immunizations (on or after 1st birthday)
    • One (1) TB skin test within the previous year
    • Three (3) Hepatitis B immunizations
    • Varicella (chicken pox) vaccine*

    Prior to the first day of 1st Grade, you will need:

    • A well-child physical taken after March of PreSchool year and before December of 1st grade.
    • All immunizations listed above for Kindergarten, except Varicella (chicken pox).

    When registering for 7th Grade, you will need:

    • Three (3) Hepatitis B immunizations (or a 2-shot series for 11-15 year olds can meet the requirement depending on the dosage your doctor provides).
    • All immunizations listed above for Kindergarten entry, except Varicella (chicken pox)
    • A Td booster is recommended, but not required.

    **Health Examination form: Included is a copy of the Report of Health Examination for School Entry form. This will be required by 1st grade, however, feel free to have it filled out by your physician any time your child has a health exam 18 months prior to entering 1st grade. Exemption provisions of the California School Immunization Law for medical reasons or personal beliefs apply. Contact the school office for exemption information.

    * Varicella (chicken pox) vaccine of proof of disease is required as of July 2001.
